Summary of Duties: Performs professional physical therapy services for the care of patients, in the patient's home; performs related work as required. Support agency philosophy and purpose.

This is a PRN position for our growing Home Health Department. Hours will vary based on patients needs.

SKILLS/REQUIREMENTS: The Physical Therapist will develop and carry out an individualized program of physical therapy including assessment, treatment planning and implementation and education to work towards a patients functional goals. The Physical Therapist will work with other members of the healthcare team to ensure excellent patient care.

EDUCATION: Graduate of an accredited college with a bachelor's degree. Must have active license as an Physical Therapist in the state of Iowa. Participates in continuing education as appropriate for licensure renewal. Adheres to all state and federal regulations regarding physical therapy. Must have a valid driver's license, good driving record and a vehicle.
